What You Will Need to Bring to Your First Appointment

2. A copy of your pay stubs for the past eight weeks
3. A copy of all lawsuits or judgments against you
4. Information about your debts and expenses

The Following Must Be Completed and Ready Before Filing

• Addresses for all creditors that are not on your credit report
• Download or print our client information sheet and complete the questionnaire as accurately as possible (provide supporting documents where available)
• Pay stubs or documents related to any income received in the past six months
• Past two years' tax returns and W-2s
• List of all lawsuits against you
• Description and mileage for all motor vehicles
• Driver's license and Social Security card (or other evidence of your Social Security number)
• Proof of any extraordinary monthly expenses (medical bills, prescriptions)
• Name and address of any person you pay or are under court order to pay alimony or child support
